Patient Referrals

I will personally contact the patient to conduct an initial phone consultation in order to best understand their visual challenges and goals. If I anticipate that low vision aids or devices will be beneficial to the patient, I will discuss with them what they can expect from a low vision evaluation and schedule an in-office appointment.

A report will be sent to you following the evaluation and the patient will be instructed to retrun to you for their continued eyecare. Thank you in advance for this referral.
